THE PRIMARY HEALTH CARE TEAM

PRACTICE STAFF

The Practice employs a Practice Manager, Nurses, Phlebotomist, Secretary and supporting staff in reception and clerical duties. The Practice Manager is Ms M. Stephens. She is available to help you with any administrative or non medical aspects of your healthcare or treatment. She is also available to discuss any suggestions or complaints you may have.

PRACTICE NURSES

The Practice Nursing Team is led by Sister Sheila Evans. Practice Nurses are available by appointment for dressings, removal of stitches, ear syringing, cervical smears, contraception checks, HRT checks, immunisations and travel advice. They are also available for general advice. The nurses are responsible for running our Diabetic, Asthma and Hypertension/Coronary Heart Disease clinics and Well Person Checks.

The nurses provide a nurse triage and minor illness service in order to provide improved access to the appropriate health professional for our patients.

PHLEBOTOMIST

Our Phlebotomists take the majority of blood tests at the practice. They are available by appointment but we can only offer this service before 11 am as the samples are collectedby 11.15 am to be taken to the lab. Alternatively you may take your request form to the Walk-in Centre, Carfax Street, Swindon between 7 am and 1pm weekdays (no appointment needed).

HEALTH VISITORS

They can be contacted on 01793 762 600 between 9.00am and 10.00am Weekdays. They will advise on both preventative and general healthcare especially for babies and children under five years, the elderly and handicapped. They run parentcraft classes and a post natal support group.

COMMUNITY NURSES

The community nurses are responsible for the nursing care of patients confined to their homes including those who are terminally ill. Their telephone number is 01793 762218 (Weekdays 8.30am - 5.00pm), 01793 541111 evenings, weekends and bank holidays.

COMMUNITY MIDWIFE

The Community Midwife will make a home visit around the 3rd month of your pregnancy to take details and arrange bookings with the hospital including your ultrasound scan. She holds ante-natal clinics at the surgery most Wednesday mornings from 9.00am. She will be happy to help with arrangements for locally held ante-natal classes and answer any questions you may have.

All mothers and new born babies will be visited by the Midwife at home until baby is 10 days old or longer if necessary, before care is handed over to the health visitor. The Community Midwife may be contacted directly on 01793 604816 between 9.00am - 9.45am, Weekdays. A message will be taken at other times.

PHYSIOTHERAPIST

The Physiotherapist holds two sessions a week at the surgery. Referral through your Doctor.

COMMUNITY PSYCHIATRIC NURSE

The CPNs are part of the Community Mental Health Team. Referral through your Doctor.

CHIROPODIST

The NHS Chiropodist holds sessions at the surgery. Referral through your Doctor.

SPEECH THERAPIST
